ABSTRACT

Objective:To investigate the correlation between the imaging markers of cerebral small vessel disease (CSVD) and early hematoma expansion (HE) in patients with spontaneous intracerebral hemorrhage (sICH).Methods:Patients with sICH admitted to the Department of Neurology, the Affiliated Hospital of Qingdao University between January 1, 2015 and December 31, 2019 were enrolled retrospectively. All patients received noncontrast CT (NCCT) within 6 h after onset. Within 24 h after the initial NCCT examination, they were reexamed to determine whether HE occurred, and brain MRI examination was completed within 48 h after onset. HE was defined as the increase of hematoma volume on NCCT reexamination by >33% or >6 ml compared with the baseline. NCCT was used to evaluate the abnormal morphology and density signs, including blend sign, swirl sign, black hole sign, island sign, and satellite sign. MRI was used to evaluate CSVD imaging markers, including lacunar infarcts (LIs), enlarged perivascular space (EPVS), white matter hyperintensities (WMHs), cerebral microbleeds (CMBs), and cortical superficial siderosis (CSS). Multivariate logistic regression analysis was used to determine independent risk factors for HE. The receiver operator characteristic (ROC) curve was used to evaluate the predictive ability of imaging markers for HE in patients with sICH. Results:A total of 216 patients with sICH were included. Their age was 57±15 years, 113 (61.6%) were male, 88 (40.7%) had HE, 123 (56.9%) had NCCT signs, 122 (56.5%) had CMBs, 143 (66.2%) had WMHs, 44 (20.4%) had CSS, 25 (11.6%) had LIs, and 31 (14.4%) had EPVS. The baseline hematoma volume, blood calcium, the modified Rankin Scale score and the National Institutes of Health Stroke Scale score at admission, and detection rates of NCCT signs, CMBs, WMHs and CSS in the HE group were significantly higher than those in the non-HE group (all P<0.05). Multivariate logistic regression analysis showed that the blood calcium (odds ratio [ OR] 0.040, 95% confidence interval [ CI] 0.004-0.238; P=0.001), any NCCT signs ( OR 3.275, 95% CI 1.492-7.188; P=0.003), CMBs grade 4 ( OR 3.591, 95% CI 1.146-11.250; P=0.028), CSS ( OR 3.008, 95% CI 1.214-7.452; P=0.017), NCCT signs+ CMBs grade 3 ( OR 3.390, 95% CI 1.035-11.102; P=0.044), NCCT signs+ CMBs grade 4 ( OR 5.473, 95% CI 1.352-22.161; P=0.017), and NCCT signs+ CSS ( OR 3.544, 95% CI 1.215-10.336; P=0.021) were the independent risk factors for HE in patients with sICH. ROC curve analysis showed that the sensitivity of NCCT signs, CMBs and CSS for predicting HE were 81.8%, 64.8% and 34.1%, respectively, and the specificity were 60.2%, 60.9% and 89.1%, respectively. The predictive sensitivity of NCCT signs+ CMBs and NCCT signs+ CSS (59.1% and 30.7%, respectively) was lower than that of single imaging marker, while the specificity (78.1% and 93.7%, respectively) was higher than that of single imaging marker. Conclusions:The imaging markers of CSVD are closely associated with the risk of HE in patients with sICH. Severe CMBs and CSS are the independent risk factors for HE in patients with sICH. The specificity of NCCT signs combined with CSVD imaging markers for predicting HE is increased but the sensitivity decreased.

ABSTRACT

Objective:To investigate high resolution CT (HRCT) manifestations and clinical features of patients infected with the novel coronavirus B.1.617.2 variant. Methods:A total of 125 patients with the novel coronavirus B.1.617.2 variant in Guangzhou Eighth People′s Hospital from May 21 to June 9, 2021 was enrolled. There were 52 males and 73 females, aged from 1 to 92 years old with a median age of 47 year. The clinical features and HRCT characteristics were analyzed retrospectively. Results:Of the 125 patients, the main clinical manifestations were fever in 54 patients, cough in 50 patients, pharyngeal discomfort in 39 patients, and diarrhea in 5 patients. HRCT showed pneumonia in 96 cases, which predominantly had ground-glass opacities in 92 cases, ground-glass opacities combined with local consolidation in 22 cases, consolidation in 11 cases, intralobular interstitial thickening in 51 cases, centrilobular structural thickening in 23 cases, and cord-like lesions in 33 cases. One patient had pleural effusion, and no enlarged lymph nodes were observed in all patients. The lesions were distributed in the subpleural and/or peripheral lung in 96 cases and along the bronchovascular bundle (middle and inner zone) in 24 cases. The time interval from onset to positive HRCT was 3 (1, 4) days in 96 patients with pneumonia on HRCT. HRCT showed absorption after the imaging findings reaching the peak in 20 patients, and the time interval between the first positive HRCT findings and the peak imaging was 6 (3, 7) days in these 20 patients. Conclusions:The novel coronavirus B.1.617.2 variant has a strong infectivity, short viral latency. Lung injury is decreaseel. People of all ages are susceptible and the diseasemay have rapid pnegression. HRCT mainly shows ground glass opacities, which are more common in subpleural and/or in the lung periphery, with interlobular thickening.

ABSTRACT

Neonatal hypoxic-ischemic encephalopathy (HIE) is a major cause of neonatal mortality and neurologic sequelae. Current evidence showed that the brain-derived neurotrophic factor (BDNF) could be considered as a specific biomarker of brain injury, which could help to assess the severity and prognosis of neonatal HIE. Also, BDNF may be widely applied in clinical practice to monitor the treatment of neonatal HIE due to its important role in neuronal survival and growth. This paper summarizes the possible role of BDNF in the management of HIE from these aspects.

ABSTRACT

Objective: To comparatively analyze the effect of TACE combined with Sorafenib or Apatinib in treatment of advanced hepatocellular carcinoma (HCC). Methods: Data of 73 patients with advanced HCC were retrospectively analyzed, including 41 underwent TACE combined with Sorafenib (group S) and 32 underwent TACE combined with Apatinib (group A). The therapeutic efficacy, safety and prognosis were compared between 2 groups. Results: No significant difference of object response rate (ORR) nor disease control rate (DCR) was detected between 2 groups after 1, 3, 6 and 12 months' treatment (all P>0.05). Dose adjustment occurred in 20 patients in group A mainly due to the occurrence of hand-foot syndrome, abdominal pain and loss of appetite, and drug discontinued in 9 cases among them. Meanwhile, dose adjustment happened in 6 cases of group S, mainly because of hand-foot syndrome and proteinuria, and no withdrawal occurred in group S. Before dose adjustment, adverse events (AE) in A group was less than in group S (P<0.01). After dose adjustment, no significant difference of AE was found between 2 groups (P=0.20). The progression free survival (PFS) rate of 6, 12 and 18 months was 31.25%(10/32), 6.25%(2/32) and 3.13%(1/32) of group A, respectively, while was 39.02%(16/41), 19.51%(8/41) and 9.76%(4/41) of group S (χ2=5.18, P=0.02). Conclusion: TACE combined with Sorafenib has better effect and safety for treatment of advanced HCC than TACE plus Apatinib.

ABSTRACT

We used adsorption method to immobilize the lipase by using nano-silica dioxide as the carrier. And we also studied different immobilization conditions effects on the catalytic activity. We got optimize immobilization conditions which were as follow the lipase quantity of 28,300 U/g, temperature of 45 degrees C, pH of 7.5 and treated for 10 h. Under these conditions the immobilized lipase activity yield 3867 U/g carrier. The best reactive temperature for immobilized lipase was 45 degrees C and higher than 5 degrees C for free enzyme, and the optimal pH dropped to 5.5 compared that of free enzyme (pH 7.0). The immobilized lipase stability for thermal and pH are improved than free lipase. When temperature was below 70 degrees C the immobilized enzyme activity was over 70% than initial activity. The free lipase activity just kept original 30% at 50 degrees C. When pH was 5-8, the immobilized lipase activity was still more than 50% and the free lipase only remained 20%. When we used the immobilized neutrophil lipase catalyzing different oil to produce biodiesel such as soybean oil, rapeseed oil and waste oil, the esterification rate of rapeseed oil was the highest.

ABSTRACT

A total of 101 patients undergoing operations for malignant gastrointestinal tumors (stage Ⅱ to Ⅲ) were enrolled and randomly assigned to receive intraperitoneal hyperthermal chemotherapy plus intravenous chemical injection (treatment group, n=51) or routine intravenous chemical injection (control group, n=50). Our results indicated that the recurrence rate and the metastatic rate in the treatment group were significantly lower than those in the control group (25.5% vs. 50.0%, 13.7% vs. 30.0%, both P< 0. 05), although the 3-and 5 year-survival rates were significantly higher (both P < 0. 05). Our data suggest that intraperitaneal hyperthermal chemotherapy plus general chemotherapy after surgery for malignant gastrointestinal tumors could effectively reduce tumor recurrence and metastases and improve long-term survival.

Sixty-one patients with moderate to severe malignant ascites were enrolled and randomly assigned to receive intraperitoneal hyperthermal chemotherapy+intravenous chemical injection (treatment group; n=31) or routine intravenous chemical injection (control group; n=30). Short-term response and reverse effects were observed. Our results indicated that the complete remission rate, part remission rate, and clinical benefit rate in the treatment group was significantly higher than those in the control group (38.71% vs 13.33% ,41.94% vs 16. 67%, and 90.32% vs 66.67%, respectively). No difference in reverse effects was found between the two groups. Our data suggest that intraperitoneal hyperthermal chemotherapy plus general chemotherapy may effectively control the malignant ascites, and the reverse effects might be well tolerated.

ABSTRACT

OBJECTIVE:To establish a HPLC method for the determination of quercitrin in Saxifrage.METHODS:The analysis was performed on C18 column(150mm?4.6mm,5?m),the mobile phase was MeOH-0.2%H3PO4 (45∶55)with a flow rate at 1.0ml/min and wavelength at 350nm under room temperature.RESULTS:There was a good linear relationship when the sample size of quercitrin was at a range of 0.40?g~2.00?g(r=0.9 996),the recovery was 95.33%with RSD at 2.80%.CONCLUSION:This method was simple, stable, fast, and reproducible and without the interference of impurity, which can be used for the content determination of quercitrin in Saxifrage.
